Tax Forms

The following are some commonly used tax forms. Hopefully these will help you in preparing your tax related material. Please don’t hesitate to contact us if you have any questions.

  • T2200 – To be filled out by an employer so an employee can claim employment related expenses on his/her personal tax return.
  • T777 Details – Employees use this form to claim employment related expenses as indicated on their T2200.
  • T2125 – Self employed individuals use this form to claim business expenses as a propietor on their personal tax return.
  • Automobile Benefit – When an employee uses a business vehicle for personal use, this form will help calculate the taxable benefit to be included on the employee’s T4 or T4A.
  • Business Consent Form – Fill out this form to give Lockhart & Company authorization to deal with the Canada Revenue Agency for your business.
  • Authorizing or Cancelling a Representative – Fill out this form to give Lockhart & Company authorization to deal with the Canada Revenue Agency for you personally.
  • T5018 – Fill out this form if you are in the construction industry and have made payments to subcontractors.
  • TD1CDN, TD1AB – Employers have this form filled out by new employees to determine the proper amount of tax deductions to be remitted.
  • CPT1 – Use this form to request a ruling from the Canada Revenue Agency to determine whether or not you are an employee or subcontractor and whether or not you should be remitting CPP and/or EI.
